The California Highway Patrol was Tuesday seeking witnesses to a four-vehicle chain-reaction crash on the San Bernardino (10) Freeway in West Covina that left one man dead and five others hurt.
The crash occurred about 5:30 p.m. Monday in the number 2 lane on the westbound 10, just east of Holt Avenue, the CHP reported.
According to the accident report, 60-year-old Amado Santos of Sun Valley was driving a 2005 Chevrolet Express van in the number 2 lane at about 60 miles per hour when the vehicle slammed into the rear of a 2014 Smart compact car driven by Yuan Zhang, 37, of Chino Hills.
Zhang’s car then collided with a 2008 Chrysler Aspen SUV driven by Jorge Vidana, 43, of La Puente, which then hit a Kia Stinger sedan driven by Jaynene Owens, 40, of Covina.
The force of the crashes fatally injured a 28-year-old passenger in the Smart car. The name of the man, who was pronounced dead at Pomona Valley Hospital Medical Center, was withheld pending notification of his family.
Zhang suffered a blunt force trauma injury, Santos was treated for a cut to his chin, Vidana had shoulder and leg injuries, and Owens and her passenger were hospitalized with shoulder, neck and back pain, the CHP reported.
All westbound lanes were closed east of Holt Avenue until about 9:30 p.m.
